We believe all consumers should be treated as individuals where the focus is ability not disability. All consumers have the same rights as non-consumers and should be encouraged to provide input into their person centered plan. We ensure to provide consumers with opportunities to be contributing members of society. CMS Agency, Inc. will advocate on behalf of the consumer to embrace inclusion in the natural environment. CMS Agency, Inc. understands the impact the family has on each consumer and we strive to educate and respect the role of the family as the primary caregiver/teacher. Staff are trained in First Aid, CPR and EBPI. Services provided through the Innovations Waiver are provided at no charge to the consumer or family. CMS Agency has been nationally accredited through CARF since 2009. We are proud to provide services 24 hours a day to you and your family in your community!

Community Living SupportsProvide instruction and assistance to enable the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community. Support, supervision, and engaging participation with eating, bathing, dressing, personal hygiene, and activities of daily living. Services are provided 7 days a week from 7am-9pm..

RespiteProvides periodic relief for the family or primary caregiver. This service may be provided in the individual's home or in an out-of-home setting. Services are provided 7days a week, 24 hours/day.
Day Supports Provides a group, facility-based service that provides assistance to the individual with acquisition, retention, or improvement in socialization and daily living skills and is one option for a meaningful day. |
Supported LivingProvides a flexible partnership that enables a person/s to live in their own home with support from an agency that provides individualized assistance. The service includes but is not limited to direct assistance as needed with activities of daily living, household chores essential to the health and safety of the individual/s, budget management, attending appointments, and interpersonal and social skills building to enable the individual to live in a home in the community.
